ABSTRACT

Radiation-induced lung injury (RILI), one of the common complications caused by radiotherapy, encompasses two phases: an early phase known as radiation pneumonitis (RP) and a late phase called radiation fibrosis (RF), threatening the life and life quality of patients, with poor prognosis. Accumulating evidence has shown that the occurrence of RILI is related to a variety of cytokines and signaling pathways. This paper summarized the research on the effects of Chinese medicine on RILI from the perspective of cytokines and signaling pathways. Cytokines include transforming growth factor-β1 (TGF-β1), interleukins (ILs), tumor necrosis factor-α (TNF-α), platelet-derived growth factor (PDGF), vascular endothelial growth factor (VEGF), and high mobility group box-1 (HMGB1). Related signaling pathways are phosphatidylinositol-3-kinase/protein kinase B(PI3K/Akt) signaling pathway, mitogen-activated protein kinase (MAPK) signaling pathway, Wnt/β-catenin signaling pathway, Notch1/Jagged1 signaling pathway, and nuclear factor-E2-related factor2/antioxidant response element (Nrf2/ARE) signaling pathway. Cytokines may interfere with RILI progression by initiating various downstream signaling pathways, such as TGF-β1/Smads signaling pathway, TGF-β1/VEGF signaling pathway, TNF-α/nuclear factor-κB (NF-κB) signaling pathway, and HMGB1/Toll-like receptor 4 (TLR4) signaling pathway. In recent years, many scholars have attempted to delay RILI progression by down-regulating the expression of cytokines, antagonizing the effect of cytokines or regulating signaling pathways. It has been verified that many Chinese medicines, Chinese medicine monomers, and compound Chinese medicine prescriptions can inhibit the release of some cytokines or regulate some signaling pathways to reduce the incidence/severity of RILI, with satisfactory therapeutic effects, which have attracted the interest of scholars.

ABSTRACT

Objective To investigate the efficacy of auricular acupressure combined with drugs in parturient woman with postpartum hemorrhage after secondary cesarean section. Methods A total of 75 parturient women with postpartum hemorrhage after secondary cesarean section,selected from January 2015 to April 2017,were randomly divided into 3 groups. Control group was given intrauterine injection of oxytocin 20 IU and intravenous infusion of oxytocin 20 IU, as well as sublingual carboprost 1 mg; combined group was given intrauterine injection of hemabate 250 μg on the basis of control group; auricular group was treated with comprehensive therapy (auricular point sticking on the basis of combined group). Postpartum 2 h, 24 h bleeding, postpartum 1 d, 3 d, 5 d height of fundus of uterus were recorded, and the initial time of lactation and postpartum 48 h lactation amount scores were observed. Results Postpartum 2 h, 24 h bleeding in combined group (152.3 ± 31.1 ml, 326.7 ± 80.2 ml) and auricular group (144.7 ± 35.6 ml, 266.5 ± 62.3 ml) were significantly less than those in control group (240.4 ± 52.5 ml, 374.9 ± 74.6 ml, P<0.05). There were no statistically significant in postpartum 2 h bleeding between combined group and auricular group (P>0.05), and the postpartum 24 h hemorrhage in auricular group were significantly less than the combined group (P<0.05). Postpartum 1 d, 3 d, 5 d height of fundus of uterus in combined group (12.6 ± 1.5 cm, 10.8 ± 1.6 cm, 9.5 ± 1.8 cm) and auricular group (11.4 ± 1.7 cm, 9.5 ± 1.3 cm, 8.4 ± 1.5 cm) were significantly lower than those in control group (13.7 ± 1.9 cm,11.7 ± 1.6 cm, 10.5 ± 1.4 cm, P<0.05), which in auricular group were significantly lower than those in combined group (P<0.05). The initial time of lactation in combined group (30.44 ± 3.64) h, and auricular group (26.36 ± 3.95) h were significantly less than those in control group (34.84 ± 6.02) h, (P<0.05), which in auricular group were significantly less than in combined group (P<0.05); the postpartum 48 h lactation amount scores in combined group (1.84 ± 0.75) and auricular group (2.12 ± 0.74) were significantly higher than those in control group (1.36 ± 0.81), (P<0.05), which in auricular group were significantly higher than those in combined group (P<0.05). Conclusions Comprehensive therapy could reduce postpartum hemorrhage, promote uterine involution and improve postpartum lactation in parturient woman with postpartum hemorrhage after secondary cesarean section.

ABSTRACT

Objective To study the effect of blood glucose fluctuations on the prognosis of thrombolytic therapy in patients with acute cerebral infarction. Methods A total of 83 consecutive patients with acute cerebral infarction admitted to the Department of Neurology,General Hospital of Beijing Military Command ( the Affiliated 81st Brain Hospital ) from January to November 2013 were enrolled retrospectively. They were divided into cerebral infarction with type 2 diabetes mellitus group (DMCI group,n=47) and cerebral infarction without type 2 diabetes mellitus group (NDMCI group,n=36) according to whether they had diabetes mellitus or not and the results of oral glucose tolerance test at day 7 after admission. Continuous glucose monitoring system ( CGMS) was used to monitor glucose for 72 hours at day 7 after admission. The mean blood glucose, standard deviation of blood glucose level, mean blood glucose fluctuation,and hemorrhagic transformation during the follow-up period,as well as vascular recanalization were observed and compared. At day 90,the modified Rankin scale (mRS) score was used to evaluate the prognosis of the patients. Results ( 1 ) Comparing the dynamic glucose parameters of the patients with acute cerebral infarction in both groups,the mean blood glucose,standard deviation of blood glucose level,mean blood glucose fluctuations at 24 hours in patients of the DMCI group were higher than those of the NDMCI group ( 8 . 3 ± 2 . 6 mmol/L vs. 5 . 8 ± 1 . 3 mmol/L,2. 1 ± 0. 4 mmol/L vs. 1. 6 ± 0. 6 mmol/L,4. 3 ± 0. 8 mmol/L vs. 3. 6 ± 0. 5 mmol/L). There were significant differences (t=31. 419, 15.537,and 15. 372,respectively;all P<0. 01). (2) Four patients (8.5%) in the DMCI group had hemorrhagic transformation during the follow-up period,17 cases (36. 2%) had good recanalization,and 15 cases (31.9%) had good prognosis (the mRS score < 2 at day 90);1 patient (2.8%) in the NDMCI group had hemorrhagic transformation,21 patients (58. 3%) had good recanalization,and 21 cases (58. 3%) had good prognosis. There was significant difference between the recanalization after thrombolysis and the prognosis in patients of both groups (P<0. 05). Conclusion The great fluctuations of blood glucose in acute cerebral infarction patients with type 2 diabetes mellitus may be an important factor of affecting its prognosis of thrombolytic therapy.

ABSTRACT

Objective To validate the clinical performance of a domestic fibrinogen reagent by freezing method(Clauss method) on the coagulometer Japan Sysmex CA1500.Methods The domestic fibrinogen reagent as the reagent A and the Germany Siemens Dade thrombin reagent as the reagent D,the Clauss method was adopted to measure the within-run precision and between-run preci-sion in two levels of quality control respectively.The reference value range was verified by the reagent A in 165 cases of normal clin-ical samples.The fibrinogen detection results in 200 cases of clinical samples were compared between by the reagent A and the rea-gent D.The significance test and the equivalence test were performed.Results The within-run precision CV of the reagent A and D in two levels of quality control were 4.28%,6.98% and 3.45%,5.22% respectively,the between-run precision CV of the reagent A and D in two levels of quality control were 6.23%,10.34% and 6.20%,9.89% respectively,the differences had no statistical significance(P >0.05).The reference value range of the reagent A was 2.08 -3.92 g/L.The fibrinogen detection results of the clinical samples by the reagent A and D had the statistically significant differences (P =0.025).But the 90% bilateral confidence in-terval(90%CI :-0.09,0.15)of the difference in the mean detection results by these two reagents located in the equivalent interval (-0.27,0.27).Conclusion The domestic fibrinogen reagent for Clauss method has reliable detection results and is suitable for the coagulometer Japan Sysmex CA1500,which is equivalent to the clinical application of Germany Siemens Dade thrombin reagent.

ABSTRACT

Objective To observe the impact of diabetes and stress hyperglycemia on thrombolytic effect and short-term prognosis in patients with acute cerebral infarction. Methods A total of 127 patients with acute cerebral infarction (≤4. 5 h) who received thrombolytic therapy with alteplase at General Hospital of Beijing Military Command from January 2012 to August 2013 were enrolled retrospectively. They were divided into three groups:Diabetes group (n=35),stress hyperglycemia group (n=49),and normal glucose group (n=43) according to whether they had a history of diabetes,random glucose on admission, and oral glucose tolerance test at day 7. At 24 h after thrombolysis,the National Institute of Health Stroke Scale (NIHSS) scores,recanalization rate,and the modified Rankin Scale (mRS) scores at day 90 were compared between the 2 groups. Results Before thrombolysis,the NIHSS scores of the diabetic group, stress hyperglycemia group,and normal glucose group were 14. 2 ± 5. 1,12. 8 ± 5. 6,and 13. 0 ± 4. 6,respectively (P>0.05);at 24 h after thrombolysis,they were 14.7 ±6.0,11.9 ±4.9,and 8.0 ±2.9,respectively (P0. 05);the NIHSS score of the normal glucose group was lower than before thrombolysis. There was significant difference (P <0. 05). After thrombolysis,the patients with good recanalization were 54. 3% (n=19),57. 1% (n=28),and 67. 4% (n=29),respectively in the three groups;the hemorrhagic conversion rate was 14. 3% (n=5),6. 1% (n=3),and 2. 3% (n=1),respectively. There were no significant differences. At day 90 after thrombolysis,the mRS scores in the 3 groups showed that the good prognosis rate of the normal glucose group was 72. 1% (n=31);it was significantly higher than 51. 0% (n=25) of the stress hyperglycemia group and 29. 6% (n=10) of the diabetes group. There were significant differences (P<0. 05,P<0. 01). There was also significant difference between the stress hyperglycemia group and the diabetes group. Conclusion Diabetes and stress hyperglycemia have varying degrees of adverse effects on the efficacy and prognosis of the thrombolytic therapy for acute cerebral infarction.

ABSTRACT

Aim To study effects of loganin and morro-niside on aging astrocytes induced by D-galactose. Methods Cortex astrocytes of newly born rats were cultured in vivo and indentified by immunofluorescence method.Firstly,appropriate D-galactose concentration was selected and effects of loganin and morroniside on proliferation activity of aging astrocytes induced by D-galactose were determined by MTT test.Then SOD, MDA were taken as indicators to study the effects of loganin and morroniside on aging astrocytes induced by D-galactose.Thirdly,growth factors like gliar cell line-derived neurotrophic factor (GDNF),basic fibro-blast growth factor (bFGF)tested by ELISA method and expression of Bax,caspase-3,phospho-extracellu-lar signal-regulated kinese (p-ERK1 /2),phospho-mi-togen-actived protein kinase /extracellular signal-regu-lated kinase kinase (p-MEK1 /2)were taken as indi-cators to discuss the potential protection mechanism. Results Loganin and morroniside exerted certain effects on proliferation of aging astrocytes induced by D-galactose,improving SOD,GDNF,bFGF release and lowering MDA release significantly (P <0.05 ). The expressions of Bax and caspase-3 proteins had no difference between model group and loganin group, morroniside group.While the expressions of p-ERK1 /2,p-MEK1 /2 of loganin group,morroniside group were improved significantly compared with model group.Conclusion Loganin and morroniside have protective effects on aging astrocytes induced by D-ga-lactose,and increase the proliferation ability.Protec-ting their antioxidant systems and improving the expres-sion of p-ERK1 /2,p-MEK1 /2 proteins are possible mechanisms.

ABSTRACT

Objective To investigate the impact of percutaneous coronary interventional (PCI) on the inflammatory indices and postoperative vascular restenosis.Methods This study involved 90 patients undergoing PCI procedures for Coronary artery disease (CAD) compromising a single coronary artery.Fourty healthy individuals with normal findings by coronary angiography were selected as the control group.Before and after PCI or coronary angiography,plasma hs-CRP and IL-6 were measured in all the subjects by immunonephelometry and enzyme-linked immunosorbant assay (ELISA),respectively.Results (1) In the CAD patients,the plasma hs-CRP level was significantly elevated after PCI as compared with the preoperative level((18.69 ±5.14) mg/L vs (14.45 ± 4.32) mg/L,t =1.42,P < 0.01),whereas in the control group,the hs-CRP level underwent no significant changes after coronary angiography((13.59 ±5.99) mg/L vs(12.46 ±5.35) mg/L,t =1.25,P > 0.05).(2) PCI procedures also resulted in significant elevation of plasma IL-6 level in the CAD patients((1.87±0.45) pg/L vs (1.35 ±0.39) pg/L,t =1.33,P<0.01),but in the control group,IL-6 showed no significant variation after coronary angiography ((1.32 ± 0.41) pg/L vs (1.21 ± 0.38)pg/L,t =1.16,P > 0.05).We observed significant difference of hs-CRP and IL-6 levels between the CAD patient group and the control group (t =4.96,6.61 respectively,P < 0.01).Conclusion Plasma hs-CRP and IL-6 are elevated in CAD patients following PCI procedures.But the roles of elevated hs-CRP and IL-6 in the vascular restenosis following the procedures need further investigation.

ABSTRACT

Objective To analyse the correlation between hepatitis B virus infection in mother and immune effect of hepatitis B vaccine in infant so as to explore ways to prevent mother-to-infant transmission.Methods 8 022 aged from 7 months to 2 years old children and their mothers were selected.The children's HepB immunization were investigated.The serological investigation of mother and children were tested by the colloidal gold tripes and ELISA methods.The HBV genotype were detected among HBsAg positive mother.Results The mother's carry rate of HBsAg was 2.43% while the children's was 0.45%.The protect rate of HepB was 81.48%.127 genotype C were detected among 146 HBsAg positive mothers.There were 26 pair of mothers and their children whose's HBsAg were both positive.Nine of the mother's HBeAg and HBcAb were positive.While five of the mother's HBeAb and HBcAb were positive,and ten of the mother's HBcAb were positive.The differences of the three were statistically significant (?2=6.03,P

ABSTRACT

Objective To investigate the effects of substance P (SP) pretreatment on the expression of β1-adrenoreceptor(β1-R) induced by norepinephrine (NE) in cultured neonatal rat cardiomyecytes.Methods The cardiomyocytes obtained from 1-3 day old SD rats were cultured for 72 h.The experiment was performed in 2 parts.In part 1 the cells were seeded in 15 well plate and randomly divided into 5 groups (n=3 wells each):control group (C1)and 4 NE groups were exposed to NE 10-9, 10-8,10-7,10-6 mol/L respectively (NE1,2,3,4).In part Ⅱ the.cells were seeded in 12 well plate and randomly divided into 4 groups (n=3 wells each): group I control (C2): group II NE 10-7 tool/L; group Ⅲ (SN)was pretreated with SP 10-6 mol/L 30 minbefore NE 10-7 mol/L and group IV (NSN)was treated with NK-1 receptor antagonist (NK-1 tachykinin receptor antagonist,S3144) 30 min before SP pretreatment.After exposure to NE for 3 h the expression of β1-R in the rat cardiomyecytes was detected using flow cytometry.Results In part Ⅰ the expression of β1-R was significantly higher in group NE1-3 than in control group (C1),with the highest expression in group NF3.In part Ⅱ the expression of β1-R was significantly higher in group Ⅱ (NE) than in control group (C2) while lower in group Ⅲ (SN) than in control group Ⅱ.(NE).There was no significant difference in the expression of β1-R between group Ⅰ (C2) and group Ⅲ and Ⅳ (NSN).Conclusion Substance P pretreatment can inhibit the up-regulation of β1-R expression in cultured rat cardiomyocytes induced by norepinephrine through activating NK-1 receptor.
